Wellness Director – Bay City Location: Bay City, MI Job Type: Full-Time About Us The Willows of Bay City is a compassionate and resident-centered assisted living community dedicated to enhancing the lives of older adults. We believe in promoting independence, dignity, and holistic wellness through personalized care and engaging programs. Position Overview We are seeking a dedicated and experienced Wellness Director to lead our health and wellness initiatives. This role is responsible for developing wellness programs that support physical, emotional, and social well-being. Key Responsibilities

• Develop and manage individualized care plans for residents
• Ensure compliance with state and federal healthcare regulations
• Monitor resident health and coordinate medical interventions
• Design and implement wellness programs and activities
• Serve as a liaison between residents, families, and healthcare providers
• Maintain accurate health records and documentation

Qualifications

• Licensed Practical Nurse (LPN)
• 3–5 years of experience in senior care or assisted living
• Strong leadership and team management skills
• Knowledge of geriatric care and wellness principles
• Excellent communication and organizational abilities
• Familiarity with electronic health records (EHR) systems

Preferred Skills

• Certification in gerontology or wellness programming
• Experience with dementia and memory care
• CPR and First Aid certification

Working Conditions

• May include evening or weekend hours
• On-call responsibilities may be required
• Physical activity including walking, standing, and occasional lifting
